Do I qualify to apply for a vehicle?

This form is designed to determine if you qualify to receive an application to apply to receive a car from our Cars Ministry. Unfortunately, because of the volume of requests we receive, we can only consider application requests from single parents with school-age children who permanently reside in one of the townships we serve, who also have no vehicle at their current address(working or otherwise, including other family members, roommates, etc), and no access to public transportation(1 mi from a bus stop).
