Princeton Alliance Church, together with their respective officers, employees, volunteers, leaders, and its parent organization, the Christian & Missionary Alliance, are designated by the abbreviation PAC, individually and collectively.
I/we, the custodial guardian(s), give permission for our child(ren) to attend and participate in activities (including water activities). I agree to assume and accept all inherent risks and hazards. I release, forever discharge, and agree to defend and hold harmless PAC from any and all liability, claims, or demands for personal injury, sickness or death as well as property damages or losses. I understand that I am responsible for any claims or losses incurred by PAC as a result of the negligent, willful or intentional acts of my child(ren) including any expenses incurred. I also understand that I am expected to stay on site and that I am not permitted to leave the premises without my child(ren).
The safety of your child(ren) is important to us. All of our adult volunteers are thoroughly background-checked every year. By hitting the "Submit" button below and by checking your child into any of our PAC related programs or services, you agree to allow PAC to document photos and/or videos for promotional use. We refrain from identifying children by name when using such content for promotional use.
I understand I am signing on behalf of the child(ren) listed on this form. My consent represents agreement of all legal guardians of this child. My consent is to liability, medical, and photo/video releases herein. I have read and fully understand the above Release/Waiver and fully understand that I have given up substantial rights by accepting this waiver voluntarily. Name of custodial parent/guardian who, having reviewed the terms of this release with of all those legally responsible for this student, represents the combined agreement and consent of such. By signing this waiver, I attest that all information provided is true and accurate to the best of my knowledge and belief.
